   at least 24 HOURS BEFORE your scheduled appointment.

    

3. There are TWO WAYS TO COMPLETE FORMS: hand-written or online,
   please choose whichever best suits your needs.
   
   NOTE: The forms can take around a 1/2 hour to complete.
   For IN-PERSON appointments, please read the driving directions carefully.
   The office can be tricky to find your first time.
